Social Determinants of Health (SDOH) are social risk factors that impact the health outcomes of a patient. A patient’s socioeconomic factors and a patient’s physical environment influence 50% of their overall health. These factors include a patient’s education, job status, income, food security, housing security, access to transportation and more.

Patients with unmet socioeconomic or physical environment factors are at risk for increased health disparities. As an example, if a patient does not have access to transportation, there is a higher likelihood that they will not make it to their appointment, potentially leading to poor health outcomes. That is why it’s so important to screen all patients for SDOH and consistently capture this data to identify trends and opportunities to improve the health of patients who are at risk and for population health at large.

In this short video, Linda Hyde, RHIA, discusses the importance of Social Determinants of Health and developing standards for the collection, use, and exchange of social risk data.
